This background check is for fingerprinting on physical ink cards.

You must bring:

1. A valid government issued ID (Driver's License, Passport, State ID)

2. Ink Cards and instructions from requesting agency.
We only provide FD-258 ink cards. We currently do not provide cards with the Privacy Act Statement listed on the back of the FD-258 cards, which can result in a submission rejection. Please check with your department/agency before using our cards.

3. Form of payment (Cash, Credit Card, Check) Checks payable to "UC Regents"

The cost for ink rolled prints is $26 for two cards and $10 for each additional card.

Your appointment will take approximately 15 minutes. If you are requesting 3 or more ink cards, please book 2 consecutive appointments to allow sufficient time for fingerprint processing.

Live Scan (Non-UCLA Employment)

Live Scan services for Non-UCLA employment only.

The requesting agency/department needs to provide you with the appropriate paperwork for your background check. You must contact them to verify correct background service and paperwork.

You must bring:
1. Request for Live Scan Form from requesting department/agency. The Live Scan form must have a mail code and ORI agency number.
*Please do not use the UCLA Live Scan Form

2. A valid government issued ID (Driver's License, Passport, State ID)

3. Form of payment (Credit Card, Cash, or Check) Checks payable to "UC Regents"

The cost for Live Scan services includes a rolling fee (which varies per Live Scan location), plus applicable State and Federal charges. The rates are currently as follows:

•Rolling fee: $26
•DOJ (Department of Justice) charge: $32
•FBI (Federal Bureau of Investigation) charge: $17

All rates are subject to change depending on the type of application being processed. You will be advised of the cost once you arrive and information provided on your form has been entered into the computer.

Your appointment will take approximately 15 minutes.

The whole process is so simple and efficient. UCPD staff is professional, kind and gentle when we did the finger scan. Street parking in front of the building helps a lot, esp in busy time.

The first time I arrived for my appointment, I was unfortunately met with a rather blunt staff member who was not the friendliest in telling me I couldn't complete my appointment that day because I didn't have my paperwork. I rescheduled for today, and a different staff member who helped me out was the absolute kindest! :)
